Withdrawal/Refund Form

We are now past the deadline for refunds, and no further refunds will be issued.

If your refund request adheres to the policy outlined below, the refund will be credited to the original credit card used for payment. For payments made by cash or cheque, a refund cheque will be sent by mail.

Administrative Fee: A $25 administration fee will be applied to all withdrawals.
U6-U9 Intro to Hockey: Refunds are available for withdrawals requested before October 30th.
U11 to U18: Once a player has been drafted to a team, refunds will not be issued, except in the case of AAA/AA players attending second tryouts. In such instances, the player’s roster spot will be held, and if the player is selected for a team elsewhere, a refund will be issued only after we receive the transfer request from a Minor Hockey Association.
Administrative Fee: A $25 administration processing fee will be applied to all withdrawals.
Tryout Fees: Non-refundable.
Late Fees: Non-refundable.
A 3% administrative fee applies to all credit card transactions. This fee is non-refundable and is charged on both the original CC payment and any refunds issued to a credit card.
NO REFUNDS will be issued to participants who choose to leave to play non-sanctioned hockey.
